Driver card - information is provided by Fructus Transport

The driver card is a personal document, issued at the request of a person who intends to use vehicles equipped with a digital tachograph in his work. Who is issuing it? What is needed to apply for it?

Who issues the driver card?

The driver card is issued by the Polish Security Printing Works. It is required for all drivers of heavy goods vehicles with a GVM of more than 3.5 tons.

What is a driver card?

The card is a medium for recording driver activity data. It is indispensable during his work - to operate the tachographs.

The driver card can be inspected, which is why it is so important to document all activities performed on the current day and during the previous 28 days on it.
 
Applying for a card

The driver card is assigned to a specific driving license number. It is the driver's responsibility to apply for it in the following situations:

- before starting work with the use of a vehicle with a tachograph installed,
- when the card's expiry date is approaching,
- when the card is stolen, damaged or lost,
- if the driver has changed his personal data or his home address.

The application for a card must be accompanied by:

- a photo on a white background showing the left half-profile or the face of the person in front of it, without a hat and glasses with dark lenses,
- signature scan - a handwritten signature made in black in a 10 cm x 30 cm frame on a white A4 sheet (the form can be downloaded from the website),
- a scan of the statement on the normal place of residence - a statement confirming that you are staying for over 185 days in Poland,
- scan of the statement on the possession of a valid driving license - statement confirming the possession of a valid driving license of at least one category: B, B + E, C1, C, D1, D, C1 + E, C + E, D1 + E, D + E ( available for download on the website),
- a certificate of reporting the theft of the card to the Police - if the card has been stolen,
- declaration of card loss - if the card has been damaged,
- authorization to submit an application on behalf of an employee - if the application for a card is submitted by the employer on behalf of his employee,
- a copy or scan of the driver's certificate - in the case of a driving license issued outside the EU and AETR,
- driving license scan - if the document was issued in a country other than Poland,
- driver card scan - if the card was issued in a country other than Poland and the driver applies for its extension.

Fee for issuing the card

In the process of applying for a driver card, one cannot forget to pay the fee for its issuance in the amount of PLN 172.20 (PLN 140 net). It is issued for a period of years.
